Monitor

Monitor is an award winning, low cost and highly scalable way of monitoring patient's well-being in the community.

Monitor provides clinicians with the same information that is provided by traditional telehealth hubs. However, unlike traditional telehealth it does not require any equipment to be purchased or installed, because all the information is collected using the phone, either a landline or mobile phone. Similarly there is no need to train the patients as they are already familiar with the technology.

Monitor is already being used to monitor the wellbeing of patients with COPD and Heart Failure, and is currently in development for patients with Asthma and for patients with Diabetes. See our News section.

Monitor automatically calls patients at pre-determined intervals (usually twice a week), and uses patient responses to pre-set questions to evaluate their state of health. Patients record their symptoms or physiological readings by responding to a series of questions using the telephone keypad. Using this data, Monitor will automatically alert the clinician if a patient's state of health has deteriorated, beyond a given threshold, using a clinician developed algorithm.

An NHS evaluation of COPD Monitor found that the “Monitor system was well received by both patients and healthcare professionals. It contributed to patients' self-management, therefore improving treatment adherence, resulting in decreased visits to hospital and to the patient's GP. The system expanded the clinical capacity of staff, by allowing them to focus on the most at-risk COPD patients, helping to improve the quality of life of these patients, as well as preventing the development of complications”.
